Services

Services can include the following:

  • Phone consultations by the hour (topics might include dementia, brain injury, stroke and end of life planning).
  • In person assessment with a written report and recommendations, addressing the 8 core areas of Aging Life Care ®. Flat evaluation fee.
  • Attend appointments, take notes, ask pertinent and follow up questions of medical providers.
  • Refer, initiate and coordination of all medical appointments, ensuring communication between all medical providers and client’s identified “team”.
  • Communication and follow up with rehabilitation or other therapists as needed.
  • Assess and advise on additional services and resources to promote safety, reduce social isolation and increase quality of life in current or future living situations.
  • Evaluate and advise on the most appropriate living setting for current and/or future needs. Collaborate with a placement agency, movers and future residence.
  • Crisis management at hospital and follow up supports.
  • Medication management, if appropriate.
  • Family and caregiver training.
  • End of life planning, support and resources.
  • Education and community resources on topics such as caregivers, placement agencies, medical providers, attorneys, medical alert systems, support groups etc.
